Evidence in one minute

  • Risk: axillary dissection (ALND), taxanes, regional nodal irradiation, BMI >30 and rural residence; not air travel, hypertension, diabetes, mastectomy, seroma or breast reconstruction — moderate. Chronic BCRL at 3 years ~4% after sentinel biopsy v ~26% after ALND. Koelmeyer 2022 · Boyages 2023
  • Prevention after ALND: a sleeve for the first year cuts first-year swelling (14% v 25%) — moderate, lasting effect low (revised 2026-10-09); progressive resistance exercise RR 0.68 — moderate; immediate lymphatic reconstruction 9.5% v 32% — low. Paramanandam 2022 · Chen 2026 · Coriddi 2023
  • Surveillance: measure both arms before surgery, then serially; focus on ALND and nodal radiotherapy. Chen 2025 · Boyages 2021
  • Treatment: garments first; intensive daily MLD and bandaging added no clinically important benefit (low); MLD adds nothing to the rest of decongestive therapy — moderate (revised 2026-10-09); night-time compression added to a day sleeve helps — moderate. Dayes 2013 · De Vrieze T 2022 · McNeely 2022
  • Precautions: blood draws, injections, BP cuffs, heat and air travel on the at-risk side are mostly not linked to onset — low; teach early signs and weight control instead. Hunley 2024
  • Ineffective: HBOT (low; revised 2026-10-08), taping (SMD 0.04; low), laser added to CDT (low), drugs and herbals, stem-cell lipotransfer. Gothard 2010 · Yang 2024 · Yoon 2026 · Lommer 2026
  • LVA (N-LVA RCT): primary outcome null at 6 months, no volume difference; not cost-effective at 2 years (revised 2026-10-08). See the lymphatic surgery topic. Jonis 2024 · Kleeven 2026
  • South Africa: about 67 certified therapists (2014), fewer than 10% in the public sector; no national BCRL guideline. Marco 2014

Risk and incidence

  • Burden: pooled arm lymphoedema prevalence 27% and incidence 21% in LMICs (BMI >25 OR ~2); 19.9% of 5 064 Turkish modified radical mastectomies (onset at a mean of 29 months; BMI 30–34.9 OR 6.6; retrospective). Torgbenu 2020 · Ay 2014
  • Axillary surgery: ALND v SNB adds 13.7% lymphoedema and 24% pain (67 studies); ALND or SNB with extensive nodal radiotherapy carries the highest trigger rates; ALND with immediate free-flap reconstruction 4–7% at 2 years by strict criteria. Most women keep functioning axillary nodes after ALND; lymphoscintigraphic change after ALND foreshadows lymphoedema (9% at 1 year, 25% at 2). Che Bakri NA 2023 · Boyages 2021 · Anderson 2024 · Szuba 2011 · Szuba 2016
  • Other factors: higher BMI, not activity or workload; symptomatic seroma (arm OR 7.8, chest OR 10.6); swelling at two weeks predicts BCRL; age did not predict BCRL in PREVENT (a weak review links it to severity); breast reconstruction does not raise risk (IRR 0.58) — low. Johansson 2002 · Fu 2011 · Delon 2008 · Guliyeva 2021 · Laustsen-Kiel 2025
  • Triggers: infection is the commonest recalled trigger; air travel rarely. Maher 2008
  • Risk calculators: 17 models, all at high risk of bias; use the core risk factors. Lin 2022
  • Breast oedema after breast conservation and radiotherapy: 18.7% (mostly mild, resolving); 72% by ultrasound, with signs harder to see on darker skin; only 5.8% by telephone self-report. Ganju 2019 · Chen 2024 · Cornacchia 2022 · Adriaenssens 2012

Measurement and surveillance

  • The definition decides the rate: 5-year BCRL ranged 43–94% by criterion in one cohort (41–91% at 30 months); the measurement method changes prevalence from 8% to 28%; the 2 cm and 200 mL criteria overcall. Armer 2010 · Armer 2009 · Hayes 2008
  • Baseline both arms: dominance confounds measurement, and a third of women are stronger in their "non-dominant" hand. Chen 2025 · Quinlan 2014 · Arinaga 2016
  • Bioimpedance: PREVENT compared BIS- v tape-triggered sleeves (no untreated arm) — less tape-defined chronic BCRL with BIS, low (ImpediMed-designed); a Belfast BIS pathway flagged 10.7%. TDC forearm ratio ≥1.2 is a research threshold for pre-clinical BCRL. Ridner 2022 · Darragh 2019 · Mayrovitz 2014
  • Symptoms: common (50% at 6 months) but a poor guide to lymphoedema; even 5–10% volume change brings symptoms; arm symptoms and pain drive QoL more than measured swelling. Hayes 2010 · Cormier 2009 · Hormes 2010 · Tsuchiya 2008

Prevention

  • Surgical: ILR (above); axillary reverse mapping with selective dissection 21% v 42% at 1 year — low. See the lymphatic surgery topic. Gennaro 2022
  • Prophylactic sleeves: the largest trial (307 women, Mumbai) — moderate for first-year swelling, low for lasting lymphoedema (first author funded by the sleeve maker). PLACE stopped at 53% of target (30% v 41%, HR 0.69, 0.39–1.23) and its curves came together after about 30 months, so any effect may be delay; a 2025 review finds little or no difference but double-counts one trial — low. Paramanandam 2022 · Bundred 2023 · Shahid 2025
  • Exercise: progressive resistance exercise RR 0.68 (0.53–0.88; 11 RCTs, I² 0%) — moderate; compression plus resistance training ranks highest in a network of 27 trials, with effects fading — low. Broader exercise RR 0.71 overall, but not significant for breast cancer alone (0.81) or ≥5 nodes (0.50); the largest trials are null — low (revised 2026-10-09). Lumping all "physical therapies" shows no clear effect. CALGB 70305 tested exercise with an activity-only sleeve (low); small post-mastectomy exercise trials are very low. Chen 2026 · Zhu 2026 · Plinsinga 2026 · Sun 2026 · Paskett 2021 · Hu 2026
  • Physiotherapy and bundles: post-operative physiotherapy 10.4% v 25.6% (confounded by adherence); a prevention bundle after ALND 8% v 33% at 2 years (small RCT); prevention clinics reduced referrals in audits. Executive Committee of the International Society of Lymphology 2023 · Boccardo 2009 · Lund 2011
  • MLD for prevention: a small Polish RCT reported 0% v 71% using a 5% threshold — very low; larger trials disagree. Do not use MLD for prevention. Zimmermann 2012
  • Education: improves arm function and shoulder outcomes but did not prevent swelling; online teaching was no better than a pamphlet — moderate. Shi 2023 · Byun 2022 · Ridner 2020
  • Practical package after ALND: a sleeve for the first year, progressive strength training, weight control, and surveillance from a pre-operative baseline.

Treatment

  • Compression and CDT: garments have the strongest support in early BCRL; garment-first is not shown inferior to intensive DLT (very low); full CDT beat modified CDT plus IPC (Iranian RCT, n=112). Executive Committee of the International Society of Lymphology 2023 · Dayes 2013 · Jeffs 2024 · Haghighat 2010
  • Early mild BCRL: a class 1 sleeve cut 6-month progression from 57% to 16% (70 women, unblinded) — low; 43% without a sleeve did not progress. Blom 2022
  • Night compression: LYNC (120 women) reduced excess volume by 11–15 percentage points over 12 weeks, and self-bandaging worked as well as a US$450 garment — moderate (sustained benefit low); LYMphoNIGHT agrees — low. McNeely 2022 · Borman 2026
  • Pneumatic compression: an advanced trunk-and-arm device beat a simple pump (−118 v +6 ml, 36 completers, p=0.05) — very low (revised 2026-10-08). Fife 2012
  • Exercise is safe — moderate (the pooled L-Dex −1.1 is a within-group change, not a benefit over control; revised 2026-10-08). Resistance bands during and after CDT, home weight lifting, resistance training v MLD, football, yoga, acupuncture and weight-bearing exercise did not provoke swelling; sudden extreme loads can. Hasenoehrl 2020 · Campbell 2019 · Do 2015 · Johansson 2014 · Kumar 2026 · Damato 2023 · Johansson 2005 · Bloomquist 2021 · Douglass 2012 · Giron 2025 · Johansson 2007 · Ozdemir 2017 · Korsholm-Rosfort 2020
  • Weight loss reduces both arms but not the inter-arm difference (low); 2–4% weight loss in BE-WEL did no better than arm exercise — very low. Tsai 2020 · Harvie 2024
  • LVA: in N-LVA, garment discontinuation (43%) was self-reported and not a randomised contrast; at 2 years the QALY gain was not significant, with an 8% probability of cost-effectiveness at €20 000/QALY. Jonis 2024 · Kleeven 2026 · Lee 2025
  • Self-care: older, anxious and less-educated women use fewer self-care skills. Arao 2023

Related conditions and complications

  • Stewart–Treves angiosarcoma: biopsy new purple papules or atypical "cellulitis" in long-standing lymphoedema. da Costa Vieira RA 2019
  • Venous access: PICC thrombosis in the opposite arm 38% with arm lymphoedema v 3% without (retrospective). BP cuffs on the at-risk arm are not shown to cause lymphoedema. Wei 2022 · Alamri 2016
  • Axillary web syndrome: cords follow lymphatic collectors; imaging findings are inconsistent. Leduc 2009 · Koehler 2014 · Leduc 2014
  • Nerve and shoulder: BCRL does not appear to cause carpal tunnel syndrome, and release is safe; severe arm lymphoedema is linked to rotator cuff injury. Forte 2022 · Avrahami 2004

Gaps and warnings

  • Still to come: N-LVA 2-year results and the final ILR report.
  • Several prevention trials are small, unblinded or industry-linked (sleeve maker, ImpediMed); pooled exercise benefits come mainly from small trials.
  • "Just in case" restrictions on the at-risk arm are not evidence-based.

For the therapist

  • Do: measure both arms before surgery; offer a sleeve for the first year after axillary dissection with nodal radiotherapy (moderate); prescribe progressive resistance training (moderate); teach early signs and weight control.
  • Stop: "just in case" bans on blood pressure readings, injections, heat and air travel on the at-risk arm (observational, low); MLD for prevention.
  • Measure: use the same method each time; repeat a raised BIS reading before treating, because most early threshold crossings resolve.
  • Certainty: first-year sleeve moderate; resistance training moderate; precautions evidence low.

For a vascular surgeon in South Africa

  • Few trained therapists, mostly private (SAMJ CME 2014); breast radiotherapy CME lists lymphoedema only as a late effect. A referral pathway was proposed. Marco 2014 · Wilson 2008
  • Low-cost options with trial support: an off-the-shelf sleeve, self-bandaging at night, and supervised strength training. Avoid PICCs and venous access in a lymphoedematous arm where possible.
